Hi there, I have moved to my family's old farm and there is a Lemon πŸ‹ tree there.
About 30 years old, planted next to an Orange Tree (probably too close).
Anyhoo, we have had tenants in the property for the last 4 years and the tree has been uncared for.
She use to give bumper crops multiple times a year but now she is looking a bit tired.
Producing fruit with growths (like a wart looking thing). I will post some photos (below).

Any help here would be greatly appreciated, don't know where to start

It kind of looks like something known as "Lemon Scab". I'm struggling with the same issue, but I haven't tried fixing it yet myself as I've been too busy.
I definitely recommend giving it a google. Or hopefully someone else on here might know more πŸ€”
 
2 grams/litre (approx 0.24 oz/gal) copper hydroxide applied three times over a 9 week period, thoroughly and meticulously sprayed upon the tree's foliage, branches, trunk, fruit, flowers and especially any new growth/buds/flowers, will greatly assist your plant to defend itself from that fungal disease. Answer courteousy of Gardening Australia (3 min video).
